 Important Information –

Please Read Carefully Before Completing the Form This form is to request transportation services for the 2025–2026 school year. Submitting a request does not guarantee approval. Transportation is limited, and requests will be reviewed based on eligibility criteria and availability. All requests should be submitted no later than June 20, 2025. Requests received after that date are subject to a wait list. NOTE: Each student needing transportation must be listed on this form. Only the students listed will be considered for transportation services. Students on intra or inter district transfers are NOT eligible for transportation services. Transportation approval follows California Education Code 39800.1, and the Board-adopted transportation priority is as follows: Students with Disabilities (with transportation as part of the IEP) Homeless Youth Unduplicated Pupils, including: Foster Youth, Migrant Students, Low-Income Students Students in Grades TK–6 who live outside the district’s established walking distance guidelines. Parents will be notified by July 18, 2025, regarding whether transportation will be provided for the 2025–2026 school year.

 Instructions for Completing This Form:

Enter a valid parent/guardian email address.

Select how many students you are requesting transportation for.

For each student, enter the following:

First and Last Name

Grade Level for each student for the 2025–2026 school year

School of Attendance for each student for the 2025-2026 school year

Enter the parent/guardian contact phone number.

Enter the home street address where the student(s) reside (address will be verified in Aeries).

Enter the city or town of residence.

Incomplete forms will not be processed. Please ensure that all required information is completed accurately to allow for proper review and processing of your transportation request. Incomplete or inaccurate information may result in a delay or denial of service.

Lea atentamente antes de completar el formulario Este formulario se utiliza para solicitar servicios de transporte para el año escolar 2025-2026. Presentar una solicitud no garantiza su aprobación. El transporte es limitado y las solicitudes se revisarán según los criterios de elegibilidad y la disponibilidad. Todas las solicitudes deben enviarse a más tardar el 20 de junio de 2025. Instrucciones para completar este formulario:

Ingrese una dirección de correo electrónico válida del padre/tutor.

Seleccione para cuántos estudiantes solicita transporte. Para cada estudiante,

ingrese lo siguiente: Nombre y Apellido Nivel de grado de cada estudiante para el año escolar 2025-2026 Escuela de asistencia de cada estudiante para el año escolar 2025-2026

Ingrese el número de teléfono de contacto del padre/tutor.

Ingrese la dirección de residencia del/de los estudiante(s) (la dirección se verificará en Aeries).

Ingrese la ciudad o pueblo de residencia. No se procesarán los formularios incompletos. Asegúrese de completar toda la información requerida con precisión para que su solicitud de transporte se revise y procese correctamente. La información incompleta o inexacta puede ocasionar demoras o la denegación del servicio.
